Hawaii now advises completing your pet entry applications through their online portal, the Hawaii's Pet Owner Portal (HIPOP). Avoid mailing delays by submitting your forms online.
If you prefer to mail your documents, HDOA will continue accepting the required paper forms found below for your pet’s travel to Hawaii:
- Dog & Cat Import Form (AQS-279) – Required for importing pets into Hawaii
- Form 7001 from the USDA – Required for your pet's health certification
- FAVN Report Form – For rabies antibody testing
